The Person-Centered Planning and Practice Resource Library is a collection of free resources on person-centered planning and practice across all Home and Community-Based Services service sectors in New York State. The materials are diverse - from leading research papers and policy guidance to planning worksheets and training webinars - and are organized in a user-friendly manner.

Please note that the resource library is intended to support the development of person-centered thinking, planning and practice skills and concepts. The resources provided are not intended as legal guidance or direction, and may not be directly applicable to New York State, but rather are included for informational purposes and examples of best practices.
