The New York State Department of Health (DOH) is sponsoring the Person–Centered Planning Comprehensive System Transformation Statewide Training Initiative in 2019 and 2020 to support best and promising practices in Home and Community–Based Services (HCBS) person–centered planning and practice throughout the State.

The learning opportunities and trainings offered as part of this the initiative are at no–cost for people who are providing, using, supporting, and overseeing home and community–based services in New York.

What are Home and Community–Based Services?

They include, but are not limited to: personal care, home health aides, homemaker services, adult day health care, social adult day care, respite, case management, habilitation, day treatment, rehabilitation, psychosocial rehabilitation, employment/ vocational services, and other services that reduce cost and help prevent individuals from needing nursing home or other institutional care.

DOH is also providing a free Person–Centered Planning Online Toolkit that will be located at this web address! This resource will be available to all, and includes:

  •           • Best practice examples
  •           • Planning and practice tools
  •           • Exercises
  •           • Recorded training webinars
